This policy explains how Ageless Minds Wellness™ Mental Health & Wellness collects, uses, and safeguards the information you share with us through this website and in the course of psychiatric care.

Information we collect

Through this website we collect only the information you choose to provide — such as your name, phone number, email address, and the details you include in an appointment request or message. Our website may also record basic technical data such as browser type and pages visited, used solely to keep the site secure and functioning.

Once you become a patient, we collect the clinical information needed to provide care, including medical and psychiatric history, medication history, insurance details, and treatment records.

How we use your information

Information is used to respond to your inquiry, schedule and provide psychiatric care, coordinate with other treating providers when appropriate, verify insurance and process billing, and meet legal and professional obligations. We do not sell your information, and we do not use protected health information for marketing without your written authorization.

Protected health information and HIPAA

As a healthcare practice, we handle protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Indiana law. Patients receive our Notice of Privacy Practices, which describes in detail how PHI may be used and disclosed and the rights you hold over your records. Indiana law provides additional protection for mental health and substance use records; those records are generally released only with your written consent or when disclosure is required by law.

Website messages are not confidential clinical communication

Email and web form submissions are not encrypted end-to-end and should not be used to share sensitive clinical details or to seek urgent help. Please limit website messages to scheduling and general questions. Clinical visits are conducted over a secure, HIPAA-compliant telehealth platform.

When we may share information

We may disclose information to treatment partners involved in your care, to insurers for payment and benefits verification, to service providers who support our practice operations under written confidentiality obligations, and when required by law — for example, in cases of suspected abuse or neglect, a court order, or a serious and imminent safety risk to you or another person.

How we protect information

We use administrative, physical, and technical safeguards including access controls, encrypted electronic health records, secure video visits, and staff training. No method of transmission or storage is perfectly secure, but we review our safeguards regularly.

Retention

Clinical records are retained for the period required by Indiana law and professional standards. Website inquiries that do not lead to care are retained only as long as needed to respond and then securely discarded.

Your rights

You may request access to or a copy of your records, request an amendment, request an accounting of certain disclosures, ask for confidential communication by a specific method or address, request restrictions on certain uses, and revoke a prior authorization in writing. Contact our office to make a request or to file a privacy complaint; you will never be penalized for filing one.

Children and adolescents

We treat patients age 12 and older. For minors, a parent or legal guardian generally has access to records, subject to Indiana law and any limits that protect the therapeutic relationship. This website is not directed to children under 12.

Cookies and analytics

This website uses only the cookies necessary for the site to operate. We do not use advertising trackers, and no health information is collected through cookies.

Changes to this policy

We may update this policy as our practice or the law changes. The revised version will be posted on this page with a new effective date.
